Our Primary Care Practice is the medical home for more than 10,000 patients and families, many of whom face economic hardships, including food insecurity, at home. To meet this urgent health need, our Primary Care team began assembling food boxes to distribute to families. Our caregivers identify families in need during medical appointments and arrange for a food box to be provided to them free of charge. These boxes are used to help bridge the gap between food resources from federal, state, and local food assistance programs.
We anticipate continued distribution of food boxes to about 60 families per month, reaching at least 720 families – thousands of individuals – over the course of the year. For around $27 per box, we could address our patients’ food insecurity for approximately $1,620 a month, or $19,440 for the entire year.
